Description of Item/Resource, and Quantity:  Include documentation of specific item (stock # as appropriate), cost and shipping fees from an FCPS approved vendor. *
Program Benefit:  Describe what program this item/resource would benefit.  What is the educational, social or other value of this request? *
Student Benefit:  How will this request enhance our students' education?  Define the scope of students who will benefit from this funding (number of students, grade levels, other pertinent info) *
Frequency/Length/Impact Usage:  Define how the item will be used and how it will improve education this school year and/or future school years. *
Training Commitment:  Specify training needs you or other faculty will need before using this item/resource.  Include in your explanation whether ongoing training will be necessary, logistics for training (online/remote/in person/etc) and any estimated ongoing costs. *
