This form is used to book your child a place at FLAG Club. Please make sure that you complete all sections on the form otherwise your booking will not be accepted. You will not receive confirmation of your booking. Please assume that if we do not contact you, your booking is accepted and we look forward to welcoming your child.

This form can be used for regular or ad-hoc bookings.

Any bookings for our after-school FLAG Club must be made by 9am on that day.

No bookings can be made via phone or email - this form is the only way to book a FLAG Club place.

*There are three choices of collection time for after-school club. The costs for each session per child are:

3:30pm - 4:30pm - £4.50

3:30pm - 5:00pm - £7.50

3:30pm - 5:30pm - £10.50

Please add 50p to the first hour for a child in Reception due to the earlier school finish and staff ratios
