A consistent dismissal procedure each day is helpful to your student and their teacher.  Fill in the form below with your student's dismissal routine.    

If you are using our Apple Pick UP System or Walkers (K-2), parents must have a sign for their car and/or to present during walker pick-up. 

*If this form is completed before August 24th, Apple Pick Up & Walker (K-2) signs will be available in the youngest child's classroom at the Meet & Greet.

How should your child be dismissed every day? *
Will your child be a walker or bike rider?  *
If your child is a walker, we require a guardian to pick up K-2 students. New this year, guardians will be given a sign just like Apple Pick Up to help with easy identification and student safety. If your child is a walker and in grades 3-5, they are allowed to walk home without an adult, as long we have consent. If you have a child in K-2 that you would like to walk home with a sibling in 3-5, please mark this below.

List the adults who are authorized to pick up your student. These adults must be listed in Skyward Family Access under Emergency Contacts. These adults must have a sign to present to staff if your child is in grades K-2.

If you are utilizing Apple Pick Up, read below and answer the remaining questions.
Apple Pick Up is a dismissal system that allows parents to pick up their children without coming into the
building to sign out during regularly scheduled dismissal. This plan is for parents who will be picking up their children daily. It is also for parents who pick up their children on a regular basis such as every Monday for
religious education or dance class.

How the "Apple Pick Up (APU)" system works.

1. Complete the information below which serves as your registration.
 
2.  You will receive 2 signs with your student's last name written on it at Meet & Greet on August 24th. 

3. Enter the south parking lot and pull up to the curb closest to door 9 by the Media Center. Make sure your sign is clearly visible to the staff member on duty. The staff member will alert the classroom that you have arrived for pick-up.
 
4. Your student will then be dismissed and will exit the doors near the Media Center. Under the direction of a staff member, your child will be permitted to enter your vehicle on the passenger side. Do not get out of your vehicle to assist. We will have staff members and student safeties on site who are able to assist if needed.

It is possible for other adults to be authorized to pick up your child. This adult must have an official Apple Pick Up sign in their car for your student to be released to them. All drivers must be in Skyward Family Access listed under Emergency Contacts. You will request additional signs for them below.

*Note that Woodbury is a dead-end street that is narrow. The line can be lengthy. It is important that you follow the guidance of staff, exercise patience, and be cognizant of our neighbors (do not block driveways as you wait).
