I am agreeing that I am 18 years of age or older.  Players playing that are not 18 will result in an immediate forfeit of any games they play in.   *

State Law prohibits the following items from being possessed and/or used on school property:  alcohol, tobacco products, e-cigarettes, vape pens, marijuana, or any other illicit drug.  This also includes any weapon including firearms.  I understand if caught with these items, I will be immediately suspended from the league and will abide by the State Law. *

I am confirming that I do not have any felony warrants or pending charges out for me at this time.  I am also confirming that I do not have any legal restrictions preventing me from being in a school building.   *
It is imperative that we treat the officials, table staff, school personnel, other players and spectators with respect.  Poor behavior has no place in the school, and will not be tolerated. Fighting will result in a minimum of a one year suspension.  I understand this expectation and will follow this rule.   *

I understand that any spectators are my responsibility and they must follow all league rules.  Any action including fighting by a spectator will be treated the same as if the team did it.  Children must be supervised and cannot enter any other area other than the gym and restrooms.  People can only sit in the bleachers that are pulled out, and not on top of the other sections. *

Food and drink other than water and/or sports drinks are not allowed in the gym.  It is also each person's responsibility to dispose of any trash they create.  Leaving the empty bottles in the bleachers will no longer be tolerated.  I agree that I will dispose of any trash in the receptacles and abide by the rules regarding no food in the gym.   *

I understand the Back Gym is off limits for any player and/or spectator. *

I expressly understand and agree that the Recreational Programs may present risks, both serious and minor, including but not limited to head or other injuries, loss of sight, broken bones, brain damage, paralysis, illness, and death, including any death, illness, injury and/or disease in any way related to or arising out of the novel coronavirus (COVID-19). I am responsible for researching and evaluating the risks that I may be faced with and am responsible for my actions. 

I attest that I am physically capable of participating in the Recreational Programs, have no known health restrictions that might jeopardize my safety or health or the safety or health of others while participating in the Recreational Programs, and that I have medical insurance.

I agree to forever release the Town of South Hadley, its employees, agents, board members, volunteers, assigns, and any and all individuals and organizations assisting or participating in Recreational Programs of the Town (hereinafter collectively “the Town”) from any and all claims or any nature whatsoever, rights of action and causes of action that may have arisen in the past, or may arise in the future, directly or indirectly, from any and all known or unknown personal injuries to myself or property damage resulting from or growing out of my participation in the Town of South Hadley voluntary Recreational Programs.

I hereby release, indemnify, defend, and hold harmless the Town against any and all legal claims or any nature and proceedings of any description that may have been asserted in the past, or may be asserted in the future, directly or indirectly, arising from personal injuries to myself or my child or property damage resulting from participation in the Town of South Hadley voluntary Recreational Programs.

I further affirm that I have read this Consent and Release Form and that I understand the contents of this Form. I understand that participation is voluntary and that I am free to choose not to participate in said Recreational Programs. By signing this Form, I affirm that I have decided to participate in the Recreational Programs with full knowledge that the Town will not be liable to anyone for personal injuries and/or property damage that I may suffer by participating or volunteering in the Town Recreational Programs.
