Before completing this form, please note that you must demonstrate a connection to a local CCDA leader and submit a letter of reference from a CCDA leader in order to request an international invitation to our 2020 CCDA Collective: A Virtual Gathering.  Without these two items, applications will not be considered.

We are opening our applications with limitation of 100 attendees - if you are applying on behalf of several people, please be sure to choose those who you feel have the best chance of meeting our criteria.
