Thank you for taking the time to join the PTA using our online form!  

Membership dues are $6.00 for each adult member. If two people are joining, please sign up both family members at the same time.  You can pay using Surrarrer PTA's electronic payment site or you can send in your dues to the school in an envelope marked PTA Membership. Checks should be made payable to "Surrarrer PTA."  You will be directed to the online payment site at the end of this form.

As a member, you will receive access to an electronic Surrarrer PTA Member Directory shortly after the school year begins.

The information you provide will be entered into our Google Database from which you will have the convenience of receiving electronic communication from Surrarrer PTA, which is a Google Group. With every e-mail message you will have the option to opt out of these communications.

Your membership in the PTA helps support the many valuable programs and activities that occur throughout the year at Surrarrer Elementary School. Our school’s PTA is the sponsor of many of Surrarrer’s big family events, including the Special Persons Breakfast, Skating Party and the Art Show/Ice Cream Social.  It also sponsors in-school programs such as D.A.R.E, Student Enrichment, and a visit from COSI.

Each membership includes access to the Ohio PTA and National PTA, valuable teaching and parenting resources, and PTA member discounts to well-known retailers and service providers. Please remember that to volunteer in your child's classroom you must be a member of the PTA.  Additionally, all board members, committee chairs, classroom head room parents and party volunteers must be members in good standing of the Surrarrer Elementary PTA.
