ON-LOCATION FORM: This online form gives parental or guardian permission for your child to participate in Good News Club (GNC). GNCs provide your child a fun-filled way to discover lessons in character development and biblical truths.  GNCs are a ministry of Child Evangelism Fellowship in partnership with local churches in your community - neighbors from your own backyard. Every GNC volunteer has been screened and trained to assure your child's safety and protection.

It will be your responsibility to pick up your child at the Good News Club at 4:15 p.m.  Good News Club will meet each Monday beginning April 4 and ending on May 23.

There are 6 sections to this form. It should take less than 4 minutes to complete depending on your internet connection speed. All sections must be completed to submit for your child's participation in Good News Club.
