Please provide us with details of your child's arrival and dismissal arrangements so that we may assist and direct him/her to the proper arrival/dismissal location each day.  We will also use this information in the event an emergency takes place during arrival or dismissal times.

We ask that you inform the school office if a last minute change in your child's routine occurs, even if only for one day.  Unless we hear from you directly, we will follow the dismissal information listed in this form.  

As the start of the school year nears (and when this information is available), it is important to review this information with your child.  Every child should know his/her bus number & stop, daycare provider, neighbor's name who will be picking up, etc.  If you are carpooling, it is important that we know who has permission to take your child home, in the event of an emergency or a last minute change to the regular routine.

Please fill out one form for each child you have at Clow School.  If you are not sure of your child's transportation for the 21-22 school year yet, you will have another opportunity to fill this out later in the summer.
