* BLOCK TEAMS ARE SUBJECT TO APPROVAL BY THE GRADING CONVENOR AND EXECUTIVE COMMITTEE

*ALL PLAYERS MUST BE REGISTERED INDIVIDUALLY VIA PLAYHQ, PRIOR TO GRADING FOR A BLOCK TEAM TO BE ACCEPTED

*JUNIOR BLOCK TEAMS MUST BE IN GRADES B OR BELOW

* Each team MUST have a nominated coach and manager (team contact) and a minimum of seven players and maximum of 10

*A player indicated within this block team form, has the right to opt out and elect to be included in the broader CSNC grading at any time.
