Funeral Planning Form
You are encouraged to use the Funeral Planning Guide, which is also available in both Microsoft Word and PDF formats from the church office or our website. It includes helpful questions, suggested hymns and Scripture readings, and theological statements from the Book of Order. You may fill out that form, or use this online form, to record your wishes. If you prefer to use a hard copy, please make sure a copy is on file with the church. It is good to go through this information periodically to make any updates or changes. Inform your loved ones that you have these wishes on file, along with other pre-planning documents.
