In order to receive the band, all information needs to be completed on the form and wrist bands will be sent home with your student listed below on Tuesday, May 21st.  If you have multiple students, only one wristband will be given. Please use your fast pass with the first student listed below.  If your student will be absent on Tuesday, May 21st, your wristband will located at the check-in table at the fast pass entrance. As always, your license/photo ID will need to presented in order to enter the building AND you will need to wear the wristband. The name on the wristband MUST match the photo id. Please remember that ALL adults attending must be verified on your student's contact list with a photo ID in order to enter the building.  If you need to add a contact before the event, please email Erinn Parrish or Leisha Shanklin.  All additions to contact list must be in writing. This Google Form will close at 3:00PM on Friday, May 17, 2023 to ensure that staff has enough time to process all of the bands.  Thank you for keeping our students and staff safe during our end of year parties!  We look forward to seeing you.
