Please complete this form to report your child absent from school. This form must be completed for each child being called in as absent.  If your child is missing school for a doctor's appointment, please send in a doctor's note in place of the excuse blank.  

This form must be submitted with a parent's email address. The form must be submitted within 3 days of the absence.
