Waiver 2024 Season
  • This form is an electronic waiver for all paddlers/participants.
  • Please fill one form per paddler/participant.
  • A new waiver will be requested for each calendar year you participate in.
  • For paddlers/participants under age 18, please have a parent or guardian fill out the second part of the waiver.
  • If you need a see a larger print of the waiver, please access it at: https://oysterpointdragons.org/waiver/
  • After your submission, you will have a chance to go back and edit your responses; so, if you suspect you made a mistake, you do not need to submit another one; just click on "Edit your response" after your submission.
  • Your response will be emailed to you. That email will also contain an Edit button. Check your Spam folder if it's not in your Inbox. Flag the email as "Not spam" or you can add Google Forms <forms-receipts-noreply@google.com> to your address book.
