What do you feel are the strengths of the student? Chloe is a bright, dedicated and determined student.  She is a quick learner, thorough, and has good oral communication and writing skills.  She is very attentive and excels in all academic areas.
What do you feel are the student’s weaknesses (e.g., areas that impede academic and executive functions? )  Chloe needs to be continually challenged.  She will sometimes be timid in asking for additional work. Chloe can sometimes become frustrated if she is asked to further develop or expound upon an answer or written document beyond her initial response.
How do you think the student learns best?  (What kind of situation makes learning easiest?)   Chloe can learn in multiple formats - but appears to response best to live (in person or remote, on-line) instruction.
Please describe educational skills that the student is practicing regularly, but need additional remediation:  Chloe doesn't need any remediation.  But we would like to see Chloe gain additional instruction in math, science, and reading comprehension and writing.
Does the student have any behaviors that are of concern to you or other school officials?  (If so, please describe the behavior(s).   No.
Please list any other information that would help us gain a better understanding of the student.
