Please complete this form to register any child that attended the PSR program last school year. One form will need filled out for each student.
Registration fee is $50/family. Financial assistance is available upon request. Please add "PSR Registration" to the memo on your check and/or indicate this on the outside of your envelope. Upon completion, please drop off your payment or mail to Sacred Heart Parish; 405 S Henrietta St; Effingham, IL 62401. Payment may be dropped off anytime during normal business hours (M-F 8-4:30) or be placed in the drop box at the Parish Office.
