This form is for PLAYERS (not parents) but PLAYERS to e-Check:in for the Fall Frenzy.  You only need to fill out this form one time.  YOU NEED TO CHECK IN EACH DAY IN PERSON EACH DAY @ TOURNAMENT CENTRAL.

I need help or support in another way unrelated to this tournament. (Purchasing equipment or discs so I can play more often, getting rides to and from practices or events, tips or advice so I can become a more confident player, shoes, clothing, food, someone to talk to, or anything else....)  Again all responses in this form are private and seen only by Coach Andrea.  Please include a brief description of your need and the best way for her to contact you about it.  (Email, at the event in private, cell phone, home phone between 3 & 5 pm, etc).
