Mountain View Montessori (MVM) is a Kindergarten - 7th-grade Public Charter school.

You must be the Parent or Legal Guardian of this student to proceed.
Filling out this form does not guarantee this child's enrollment.
All open spots are filled via random lottery with initial preference given to faculty students, siblings of enrolled students as set forth in our Charter.
Lottery entries are not rolled over to the next year.
Your child must be accepted from this lottery/waitlist before official enrollment is allowed.
You will be notified via email if/when your child is accepted and it will contain instructions to enroll your child/ren.
It is your responsibility to check your email regularly and to keep your contact information updated. (add

Once your child has been accepted via email from the lottery/waitlist, you have 7 calendar days to enroll them online or their spot will be auto-declined and their name will need to be re-entered on this form.
Spots/seats will not be held for tours, indecision, vacations, etc.
Our lottery is held mid-February each year. The cut off for lottery applications is the day before the lottery.

***Their Birth Certificate and Immunization record (or exemption form) must be turned in prior to enrollment. Failure to do so could result in delayed attendance. It is the parent/guardian's responsibility to provide these documents. Obtain copies from a previous school if necessary.
***Immunizations must be current and up to date before the first day of school.

Any answers or entries found to be untrue during the application, registration or enrollment process could result in denial of enrollment.
