I understand and accept the following conditions:

1. That, if I borrow a school instrument, I shall be responsible for the cost of any repairs or loss of the instrument, and I undertake to arrange appropriate insurance coverage. (All instruments should be insured. Cover for the instrument at home, en route to and from school and while at school can usually be arranged as an extension to household policies). Instrument hire is charged at £20 per term.

2. That any instruments used by my child will carry a label with their name and address inside the case. (It is also useful to attach some readily identifiable external mark - a sticker or similar - since many instrument cases look alike and mistakes are sometimes made in collecting them). Most instruments carry an identification number - this should be noted in case of loss.

3. That I must give at least one half-term's notice (or 6 working weeks), in writing or by email, of an intention to discontinue lessons. Failure to do so will result in the bill still being payable, whether or not pupils attend lessons.

4. That I will pay tuition fees (and examination fees and accompanist fees, if and when entries are made) by the due date stated on the invoice.

5. That a termly bill will be for 11 lessons and that if, for any reason, this number of lessons is not offered, then the bill will be adjusted accordingly or a refund will be issued at the end of the academic year.

6. That attendance at instrumental lessons is primarily the pupil's responsibility and that credit will not be allowed for lessons lost by pupil's non-appearance at the time designated (including pupil absence from illness and medical appointments).

7. Only lessons missed by reason of a teacher's absence or a school-sponsored event (examinations, trips, etc) will be credited, unless the teacher makes arrangements for an alternative lesson date. It is the pupil's duty to inform the instrumental teacher personally (and in advance) of trips etc.
