I consent to my child participating fully in the Technical Theatre program at Providence High school. I have read the class disclosure and give my consent to the rules, grading polices and procedures as outlined. I understand safety is extremely important and students will be severely disciplined and or removed from participation in class for unsafe behavior, including horseplay and failure to wear safety glasses. The safety in this class comes first and I will never compromise the safety of the students, through constant supervision and proper training on all tools. Please recognize that a risk of possible injury is inherent in the operation of Technical Theatre equipment (i.e. table saw, miter saw, jigsaw, circular saw, routers, drills, stage lights). I further realize that potential injuries can be severe in nature. I understand that my son’s/ daughter’s enrollment is made only upon the express condition that I shall hold harmless and indemnify the instructor and school district from all liabilities and damages by reason of any injury to my child resulting from his/her participation. I further consent to treatment deemed necessary by health care providers designated by school authorities for any illness or injuries. I understand that a copy of this form will remain on file.
