JV Pommie Apparel Order Form 2021-22
WITH A PARENT, please complete the following questions about your apparel sizes and needs.  Everything on this order form is needed for this year.  IF you already have the items below, please do not order them.  You may visit Head-To-Toe Dancewear for the sizing of any of their items.  You are responsible to pay for any items you order on this form.  Everything you order, you will keep.  After submitting, you should receive an emailed copy of your responses.  I will also send you an invoice for your order with payment due dates.  Contact Coach Bekebrede with any questions or concerns.

Team t-shirt: $10.00.  Please select your size for a unisex cut t-shirt.   *
Please type the name you want written on the back of your shirt.  Most people put their last name.  i.e. "BEKEBREDE" *
Program t-shirt: $7.00.  Please select your size for a unisex cut t-shirt.   *
Tank top: $28.00. Please select your size for a dry-fit type athletic tank top.  This is most likely a fitted, racerback cut.   *
Sweatshirt: $25.00.  Please select your size for a hooded sweatshirt. *
Leggings: $28.00.  Full length, black performance leggings from Capezio.  Please select your size legging or "I already own these." *
Dance pants: $23.16.  Black, V-cut performance dance pants from Eurotard.  (Last year's Body Wrapper pant will also work for this year.)  Please select your size or "I already own these." *
Dance sneaker: $56.00.  Black Dansneaker brand shoe, DS24.  Please select your tennis shoe size or "I already own these." *
BLACK Jazz shoes: $34.40.  We order the Capezio brand EJ2 jazz shoe.  It is acceptable to wear something similar to these.  Please select your size jazz shoe or "I already have black jazz shoes.   *
TAN Jazz shoes: $34.40.  We order the Capezio brand EJ2 jazz shoe.  It is acceptable to wear something similar to these.  Please select your size jazz shoe or "I already have tan jazz shoes.   *
Tights: $12.40.  These are Capezio- convertible, carmel colored tights.  I can approve a different color tight for darker skin tones.  Please select your size for tights or "I already have these."   *
Trunks: $8.00.  The JV team has a skirt they could wear and the red trunks are worn underneath it.  These run small.  Please select your size or "I do not need to order these." *
Poms: $30.00.  These are red and white poms that JV uses for performances and continue to use them on Varsity for practices.  Please select "yes, order these" or "no, I already have these." *
Required
Windbreaker Jacket: $35.00.  This is a red windbreaker style jacket with front pouch and embroidered pocket.  This is worn by JV and Varsity.  Please select your size or "I already have this."   *
Nike bag: $30.00.  This is a medium sized duffle bag with "Kirkwood Pommies Dance Team" screen printed on the size.  Most members buy this bag, but this is not something you have to purchase.     *
