Release of liability for Covid. *

I acknowledge the contagious nature of the Coronavirus/COVID-19 and that the CDC and many other public health authorities still recommend practicing social distancing. I acknowledge that I must comply with all set procedures to reduce the spread while attending my classes or projects at Notes n’ Beats. I hereby release and agree to hold Notes n' Beats LLC and its partner company, their staff and faculty harmless from, and waive on behalf of myself, my heirs, and any personal representatives any and all causes of action, claims, demands, damages, costs, expenses and compensation for damage or loss to myself and/or property that may be caused by any act, or failure to act of the studio, or that may otherwise arise in any way in connection with any services received from Notes n' Beats LLC.

Media, Food, Facility and NoPoach *

I agree to the following:

Media Release: Allow for photographs and videos of students and campers to be used for Notes n’ Beats and/or Science Cosmos advertising (Social media, Website, etc)

No Poach: I will not reach out to students and staff of Notes n' Beats or its partner companies for any reason to do voluntary and/or for-profit business with them. I will be liable for fines if that happens.

Food Waiver: Discharge Notes n’ Beats and Science Cosmos of any claim from food served or ordered from external vendor or medical service rendered or first aid treatment done.

Facility Use: Understand that Notes n’ Beats or Science Cosmos is not a licensed child day center and the duly signed is fully aware of this.

Signed and named below, I am parent or legal guardian of the student(s) named above (“Camper", “Participant”) and In consideration for Participant’s participation in a class, camp or event being held at Notes n' Beats or its partner (Science Cosmos) location, I, as Participant’s parent or legal guardian, on behalf of Participant, his/ her heirs, executors, and administrators, hereby voluntarily, irrevocably, and unconditionally release and hold harmless Notes n' Beats and Science Cosmos and their respective trustees, officers, employees, agents and assigns (hereinafter “Releases”), from any and all manner of claims, actions or causes of actions, whether related to or arising from the negligence of Releasees or otherwise, arising from or in connection with Participant’s participation in the Program, including but not limited to arising from Participant’s presence on the premises of Notes n’ Beats’s or its Partner (Science Cosmos) camp, Class or event location, or the Participant being transported to or from or traveling to or from the premises of Notes n’ Beats or its Partner (Science Cosmos) camp, class or event location. I fully understand that Notes n’ Beats or Science Cosmos is not a licensed child day center and the duly signed is fully aware of this. This release includes but is not limited to claims of active or passive negligence, products liability, personal injury, death or damage to property or violation of any laws or regulations.
