I agree to my child taking part in Isca HC activities at Easter camp 2021 (coaching, training, matches etc.)
I agree that I will not send my child if he/she shows any of the recognised COVID-19 signs such as cough, high temperature etc
I agree that my child will abide by all requests with regard to COVID-19, such as correct distancing, responsible for own equipment and will not touch any other equipment, agree to bring own sanitiser and wash hands frequently.
I confirm that, to the best of my knowledge, my child does not suffer from any medical condition other than those stated above.
I authorise the leader of the camp, or any other coach, to consent to such medical or dental treatment (including inoculations, blood transfusion or surgery) which, in the opinion of a qualified medical practitioner or dental surgeon, may be necessary if you cannot be contacted.
I consent to the taking of photographs/digital images of my child during club activities to be used on the club’s website and where appropriate for use in press coverage and for coaching purposes.  Images will not be named.
