Davis County Health Department can provide you with education and vaccination to help keep you healthy while traveling. You can significantly reduce your risk of illness by having a pre-travel consultation with one of our Travel Nurses.
Travel Program Details:
-Full Travel Consultation 45 mins-1 hour, plus 30 mins per additional traveler, $30 per traveler
Any Travel Prescriptions for medications needed
Yellow Fever, Typhoid (Injection and Oral), Japanese Encephalitis, and Rabies
Up-to-date information on potential health threats, including disease outbreaks and travel advisories
Tips and advice on how to remain safe and healthy during your travel
Helpful information on the local culture
Detailed information about immunizations and medications needed
Travel Consults are for any Traveler, or especially those who meet the following criteria:
-Immunizations, no consultation
No detailed information regarding the Country will be given
No Prescriptions for medications will be given
Typhoid (Injection or Oral), Japanese Encephalitis, and Rabies.
Important Appointment Information:
We do not bill insurance companies for Travel consultations or Travel vaccines. All Travel expenses must be paid out of pocket and at the time of service.
All vaccine costs can be located on our Vaccine Cost List.
If during the consultation, the nurse recommends routine (NON-TRAVEL related) vaccinations, Davis County Health Department may be able to process those costs under your insurance.
We accept cash, checks, or credit cards. If using a credit card, be advised Davis County Health Department charges a 2.50% processing fee with a minimum of $1.55.
Please print and complete the DCHD Travel Clinic Encounter Form and bring it to your appointment to expedite the check in process.
Please arrive 10-15 minutes early to your appointment.
If you are more than 10 minutes late, you will be asked to reschedule.
