Classes are held within the safety and convenience of your child’s center. By participating in our classes, you understand that there are certain risks of injury. By submitting this form you are giving full consent to your child's participation, and use of all equipment, and you waive, release and hold harmless Head To Toe, its officers, coaches, contractors, and representatives for any injury that can be incurred. INTERNAL USE PHOTO POLICY - To keep families involved, Head To Toe takes photos/videos during class which WILL be distributed to the parents of your child's specific center ONLY, they will not be used or shared in any other way. If you do not agree to this internal use photo policy you MUST notify us in writing, via the comments below or email.
