Program Type
A PAL or other program representative may represent only one type of program at one peer review session. Individuals who are PALs for two types of programs must attend two different sessions and complete this form for each type.

Program Representative
Typically, the person representing the program at the peer review is the PAL. If the PAL is unable to attend one of the peer review dates, the PAL may find an alternate to attend. Programs are also welcome to have two representatives present, which is often useful when the PAL responsibilities are transitioning from one person to another.
