I have read this document and understand that neither Tulsa Chinese School, nor the hosting Tulsa Chinese Christian Church, nor their Board members, administrators, teachers, the voluntary workers will be held responsible for any injuries, accidents or COVID-19 exposures that may occur to my child/ren during class hours or extra-curricular school activities. I have hereby freely signed this agreement out of my own will and deeds.

From time to time Tulsa Chinese School takes pictures and videos of students during class and extra-curricular activities and publishes them in public media. Check, initial and date here if you want your child/ren’s pictures /videos published in the media.
