ALMS Return to School (Four Days a Week) or Full Virtual Model Parent/Guardian Survey

Dear ALMS Community Members,
Please complete this survey form to inform us of your learning model preference for the remainder of the 20-21 school year. Please let us know if your child/children will attend Albert Leonard Middle School in-person (four days a week) for the remainder of the school year, or if you will choose to have your child/children remain in the Virtual Learning Model for the remainder of the school year.
The parent and student information entered should match the parent/guardian/student information we have on file. Please complete one survey form for each child that attends ALMS.
Submission responses are requested this week and due by Friday March 5, 2021.
Below is the anticipated timeline:
3/5/21: Survey Responses Due to ALMS
3/12/21: Planning and Adjustments to Determine Feasibility
3/15 - 3/19/21: Families Informed and Decisions Confirmed
3/22/21: Target Start Date of Combined Cohorts (Four Days a Week or Full Virtual Learning Model)
