This appointment is ONLY for those who received Sinpoharm 1st dose at Lady Ridgeway Hospital between 10th-15th May. The vaccine card will be checked at the gate for the accuracy of details. If you received the vaccination at a later date, you can get vaccinated on a later date arranged by the hospital. First dose of the vaccine WILL NOT be given.

Please read these instructions carefully.
- If there are more than one member in your family you can fill more than one form.
- You can only select the date of vaccination. Your timeslot will be allocated according to availability.
- You will get an SMS with the time of vaccination and appointment number. Please bring this SMS, Vaccination Card and NIC when you come for the appointment.
- Please arrive 10 minutes early for your appointment. If you are late than 10 minutes from the stated time you will not be admitted to the vaccine center.
- Vaccination is held at the St. Luke's Church Borella (Maradana Road) next to the hospital.

- ඔබේ පවුල තුළ එක් සාමාජිකයෙකුට වඩා සිටී නම් ඔබට එක් පෝරමයකට වඩා පිරවිය හැකිය.
- ඔබට එන්නත ලබා ගත හැකි දිනය පමණක් තෝරා ගත හැකිය. ඔබගේ කාල පරාසය හැකි ප්‍රමාණය අනුව වෙන් කරනු ලැබේ.
- එන්නත හා පත්වීම් අංකය සහිත කෙටි පණිවිඩයක් ඔබට ලැබෙනු ඇත. ඔබ එන්නත්කරණය සඳහා පැමිණෙන විට කරුණාකර මෙම කෙටි පණිවුඩය, එන්නත් කාඩ්පත සහ ජාතික හැඳුනුම්පත රැගෙන එන්න.
- කරුණාකර ඔබගේ හමුවීම සඳහා විනාඩි 10 ක් කලින් පැමිණෙන්න. ප්‍රකාශිත වේලාවෙන් මිනිත්තු 10 කට වඩා ප්‍රමාද වුවහොත් ඔබව එන්නත් මධ්‍යස්ථානයට ඇතුළත් නොකරනු ඇත.
- එන්නත්කරණය රෝහල අසල පිහිටි ශාන්ත ලූක් දේවස්ථානයේ බොරැල්ල (මරදාන පාර) හිදී පැවැත්වේ.
