MCES PTO Funding Request Form

Thank you for using our funding request form! This will help us expedite the process in getting you much-deserved support for your classrooms, students, activities, etc.! If the amount is over $50, we may request that you present at our upcoming PTO meeting. If the request is time-sensitive, we may ask that you present afterwards, if the request is approved. We would like to ensure that our parents, families & PTO members see the amazing work you're doing AND feel motivated to continue supporting our amazing staff! 

Please submit this completed form and all supporting documents at least two weeks in advance of when the items/funds are needed. We will contact you you accordingly -  if your request has been approved, you may then either request a check directly from the PTO Treasurer or make the authorized purchase and request reimbursement. All appropriate invoices and receipts must be turned in to the Treasurer after purchase. While the PTO would like to honor all requests for funds, we have a limited amount of funding to work with. The PTO gives preference to funding requests that will impact as many students, families, and faculty as possible, with consideration given to those that provide a longer-term benefit. Please recognize that every request is considered important, but a requested item or service should advance the PTO’s mission to foster the relationship between school, home, and community for the academic success of all of our students.
