We are preparing for and limiting exposure of COVID-19 in the Clay Center community by identifying healthy adults who are under 60, whose risk factors are low and who are willing to volunteer for members of our community.

Once you have filled out this form a volunteer will contact you to coordinate the delivery of essential supplies, like food and prescriptions.

NOTE: In signing up for this program, the volunteer will neither enter your home nor come within six (6) feet of you. Upon delivery and receiving of supplies, you will communicate with your volunteer by phone, who will drop supplies at an agreed upon location outside the home.
