The effective date is the date CCS will use as your last day of employment. You cannot be paid for any time worked after this date.

An entered effective date of resignation that occurred prior to the date you submit this form will be reviewed by the Director of Labor Management and Employee Relations or Designee for approval. If the entered effective date is not approved, the date this form was submitted will be used as the effective date of your resignation.

For resignations it will be the last day you either work or use approved leave (paid or unpaid).

If you intend to complete the current 2023-2024 school year and MAINTAIN benefits through July 31, 2024 your EFFECTIVE DATE must be on or after July 1, 2024.
