"SOC 2 Report") in connection with the use of Autify Services, Autify
for Web or Autify for Mobile from Autify, Inc. (US) and agree to the following terms and
conditions:

・I/We hereby
acknowledge that the SOC 2 Report is confidential information of Autify;
・I/We
will use the SOC 2 Report only for the following purposes:
- Assessment
of the security of the Autify Service; and
- Vendor
review necessary for obtaining and maintaining SOC 2.
・I/We
hereby agree not to disclose, provide or otherwise leak the SOC 2 Report to any
third party other than our directors and employees to the minimum extent for
the aforementioned purposes and to keep the SOC2 Report strictly confidential;
l I/We
will use the SOC2 Report in accordance with all the special terms and
conditions related to the use of the SOC2 Report contained in the Autify Terms
of Service (https://autify.com/terms) or other related documents.
I/We hereby agree that any personal information contained
in this request form will be handled in accordance with Autify's Privacy Policy
(https://autify.com/privacy).
