UMN SNMA Mentoring Program:       Application to be a Mentee
**NOTE: Due to a high volume of applications in recent years, this year the mentorship program will only be accepting 75 applicants. By filling out this form you are indicating that you are ready to comply with the guidelines and expectations and the required activities of the program.**

These expectations include:

-Having a serious interest in medical school and the career of a physician
-Initiating contact with your mentor
-Enthusiasm regarding receiving advice from your mentor
-Mandatory attendance at Welcome Event via zoom on Sept 23rd 2020, and Closing Event (TBD: March/April).
-Required attendance to at least one panel or workshop event hosted by UMN SNMA Mentoring program

Reminder: Mentee­-mentor relationship is not expected to be a substitute for any academic advice provided by the University of Minnesota academic counseling offices.
