Please complete one form per player. All forms and payments need to be returned by April 19th to Mrs. Mack in the elementary or Mrs. Tew in the HS/MS. Checks should be made payable to TEKONSHA SUMMER BALL ASSOCATION or TSBA. All parents, coaches, and players will be required to sign and follow a code of conduct. Failure to follow the code of conduct may result in dismissal from the team without refund. Questions should be relayed to TSBA Director Phillip Miller at (517) 317-7107.
