Please take note that all Video Conferencing must use parish Google Hangouts Meet/Google Meet using your parish email account provided to you.

Please be clear and specific with the information given or it will be rejected. This form have to be submitted at least 7 days (1 week) in advance.

Google Cloud - G Suites Applications such as Google Hangouts Meet/Google Meet allows you to access and use our parish email domain "@csfaministries.sg" for ministry/group or "@csfa.sg" office for staff. All parish internal and external communication must use parish email for official use.

In compliance with the Personal Data Protection Act (“PDPA”), The Church of St. Francis of Assisi (“SFA”) seeks your consent to collect and use your personal data (i.e. Name, mobile/WhatsApp numbers, mailing and email addresses) in order to maintain the Google Cloud G Suite registry of membership and to disclose such personal data to Church SFA affiliated organisations where necessary (“Purposes”).

By submitting this form, you acknowledge that you have read, understood and agreed to the PDPA-Policy stated.
