2024 Spring Class Registration form

By filling out and signing on this form:

1) I hereby give my approval to enroll the student on the registration form to Lone Star  Chinese School.
2) I hereby give my permission to the student/students who on the form to participate in all school activities. 
3) I hereby release Lone Star Chinese School, its teachers, volunteers, guests, other parents, and students from liability for any illness, injury, misadventure, harm of any kind suffered because of participation in Chinese school camps, classes, and all activities.
4) I acknowledge the contagious nature of COVID-19 and voluntarily assume the risk that above student/students and I may be exposed to or infected by COVID-19 by attending activities in Lone Star Chinese school. I further acknowledge that Lone Star  Chinese School will not be liable if the student on the registration form and myself or other family members are infected by COVID-19. 
5) I hereby give my permission to Lone Star Chinese School to act as guardian if I cannot be contacted in the event of an accident or medical emergency involving my child/children. Also, in the event of emergency, the school has my permission to obtain medical treatment for the proper care and well-being of my child/children, including ambulance service. All the cost is my responsibility. 
6) I hereby give my permission to Lone Star Chinese School to take photos and videos of student on the form. The photos and videos may be released to the school, or other related flyers, websites, newsletters, weekly bulletin board, school parents, etc. 


The school offers Chinese class, Arts class and Chinese & Activities class. All classes run from Jan. 6th to May 18th. Class week: 15 weeks(Saturday); 17 weeks( Monday and Friday) 18 weeks( Tuesday, Wednesday and Thursday)

Class fees: registration $20.00 per student per class; $30 per family per class; $30 after class started; Books: $25-45.00; Material if any: $20.00
Saturday: Chinese class (15 weeks): $420.00
Tuesday, Wednesday and Thursday: Chinese class (18 weeks): $490.00
Monday, Friday: Chinese class (17 weeks): $465.00
Wednesday, Saturday: Chinese Culture Arts ( drawing, painting, calligraphy....14 weeks) : $390.00

Friday: Mommy & Me class $30/hr

Monday, Wednesday, Friday: Pre-K/K: $1125/15 week once a week; or $65.00/morning; $20.00 per additional hour

After-school care: $260.00 weekly including Chinese class, math class, activities, and homework tutoring

The trial class policy:

1) The students(older than 18yrs old)  or parents must complete the registration form which includes a liability waiver.  
2)The trial class must be scheduled in advance.
3)Respectful behavior is expected from the trial class participants. This includes following instructions, being attentive, and treating classmates and teachers with courtesy.
 4)The trial class is closed to observers, including parents or other family members. This policy ensures a focused and distraction-free environment for both the trial class students and teachers. 
5) No fee for a Chinese trial class. If the trial/prospective student decided to enroll in the class, then the trial class will be counted into total class fee.
6) $25.00 for a Drawing trial Class, and $15.00 for a Chinese & Activities trial Class.

All fees are non-refundable and non-transferable. The fees are not allowed to transfer to another student, 

class, or semester.  

A 24-hour advance notice is required to make up a class. Otherwise a missed class will be charged in full amount.
