School Opening for Key Workers, Vulnerable Groups, SEND
Please complete below to the best of your ability - we know it's not easy but please follow the government guidelines as per the email.  Please give as much information as possible.  This form is for all categories - SEND, vulnerable groups and Key workers - please forgive the length but we need returns quickly so bear with it.

SEND and vulnerable groups means - any child with an Education Health and Care Plan or other SEND need as agreed with the SENCO; any child who has agreed SEMH needs; any child who has Child in Need or Child Protection Plan and any child where there is regular family support or Time to Talk support given in school (please note this is the way we have chosen to define this and it may be different in other schools)

If school was to be kept open during the Easter holidays would you need your child to attend?  Please note there is no knowledge about who this would apply to - just Key Workers; Key Workers and SEND etc - this is just a speculative question at this stage. *
Will you need extended hours - school will be open 8:30 - 3:30?  This provision will depend on staffing availability and I cannot promise this at this stage. *
