OUSD Safety Agreement- Families

OUSD Families will be oriented to the Safety Protocols required for keeping schools safe during Covid- 19 pandemic. These agreements support a hybrid reopening plan for in-person instruction.

My child/children will wear a face covering while at school, unless they have been granted a medical exemption (ask for the form if needed). *

My child/children will wash and/or sanitize their hands regularly while at school. *

I will ensure that my child’s/children's Daily COVID Health Screening Form is completed in ParentSquare every day, ideally before my child leaves for school. *

I will ensure that my child/children is tested for COVID-19, if required by the District. *

I will not send my child/children to school if any of the responses to the Health Screening Form are YES. *

I will keep my child/children home for the amount of time directed by the District if my child tests positive for COVID-19, has symptoms of COVID-19, or has prolonged close contact to someone with COVID-19. *

I understand that sometimes very mild symptoms can be a sign of COVID-19. *

I will notify my child’s principal immediately if my child tests positive for COVID-19, has symptoms of COVID-19, or has prolonged close contact to someone with COVID-19 *
