Faith Formation Registration 2023-2024
All Fees collected for Faith Formation will  go toward the purchase of books and materials.

The fee for Faith Formation for  this year is $20 per child, max at $60. 

1st day of Faith Formation– Sunday September 17, 8:45am - 9:45am –PARENTS PLEASE ATTEND

**Faith formation sessions will be in person on Sundays from 8:45am to 9:45am unless otherwise noted.

A child will not be permitted to leave with anyone other than the individuals listed above, unless the parents directly communicate to the teacher or DRE who the child will be leaving with on the specific day. Parents may modify this form at any time by contacting the DRE.
Please let us know ways in which you would be willing to assist with the Faith Formation program. We love when our families are involved! Please consider donating your time to help the children in their faith journey.

I understand that Faith Formation classes do not substitute for Sunday Mass Obligation and that I set the example for my children and their faith. I will make Sunday Mass and Faith Formation a priority in our household.
