Please fill this out at least 1 week before your anticipated absence. You will be contacted before the meeting to let know if your absence will be counted as excused or not.
If you have a test, please attach a picture of your schedule. If you have any paperwork to support your reason, please attach it.

EXCUSED ABSENCES
(ex: exams during meeting times, sick (please provide note if possible, emergencies)

UNEXCUSED ABSENCES
(ex: studying for exams, exam reviews, study groups, group projects, forgetting about meeting, outside social events, work, etc.)

Each member is permitted 3 excused and 3 non excused absences per semester. If you exceed your 3 unexcused, you will be placed on probation for the following semester. If on probation, a member not meeting their points or receiving more than 3 more unexcused absences will be subject for removal from SAM.
