City of Wellman Reservation Form
This form is to rent the shelters and ball fields in the City Parks of Wellman.

PARK RULES AND REGULATIONS
*Park reservations are made first come, first serve basis.
*Additional tables and chairs may be brought in - Provided there are no damages to park.
*Smoking is prohibited in all City facilities and Parks.
*After use, shelter area, must be cleaned from your use.
*All used trash bags must be removed and replaced with new bags located in the bottom of can, and trash bags disposed of in the dumpster.
*City parks close at 10:30PM
*All other City Rules and policies are to be obeyed.
Any damages incurred during reservation will be the responsibility of the person or organization who completed this form.  
***NO ALCOHOL TO BE ON ANY CITY PROPERTIES.***

BALL DIAMOND USAGE
*Applicant is responsible for preparation of the field.
*Applicant is responsible for keeping the ballfields and dug outs in a clean manner, leaving the park in an orderly fashion, picking up litter, etc..
*NO PETS on the fields.
*Park closes at 10:30PM and lights will be turned off then.

***NO FIREWORKS OR ROCKETS are to be set off in City Parks or Ball Diamonds.
Respect the boundaries of the park and ball diamonds and DO NOT TRESPASS onto adjoining properties.
