Complete this form to register for the Fall 2019 Nature Experience class. Due to limited space, completing the form does not guarantee a spot in the class that begins October 16, 2019. Immediately upon completing this registration form and hitting "Submit", an email summarizing your registration entries will be sent to the email collected.

IF YOU DO NOT RECEIVE THIS AUTOMATIC EMAIL, YOU DID NOT COMPLETE THE REGISTRATION.

If you register after the class is full, you will receive a separate email within a day explaining this. If you do not receive this separate email, then you have your spot in the class. Congratulations and thank you!

IF YOU ARE GOING TO MAIL A CHECK TO PBS FOR PAYMENT,  A RECEIPT WILL BE SEND WHEN THE CHECK IS RECEIVED.

3RD PARTY (FI) INVOICING:
IT IS YOUR RESPONSIBILITY TO DETERMINE WHETHER THE CLASSES ARE APPROVED FOR REIMBURSEMENT BY YOUR FI. EACH FI INTERPRETS THINGS DIFFERENTLY. THE CLASSES ARE INTENDED TO MEET A NEED IN THE COMMUNITY AND THERE IS NO GUARANTEE THAT ALL FIs WILL COME TO THE SAME ELIGIBILITY DETERMINATION.

PBS WILL SEND A SINGLE INVOICE FOR THE ENTIRE SEASON IN THE FINAL WEEKS OF THE CLASS. IF YOUR FI ONLY MAKES REIMBURSEMENTS ON A MONTHLY BASIS, YOU WILL BE RESPONSIBLE FOR SUBMITTING A COPY OF THE PBS INVOICE MULTIPLE TIMES TO THE FI. PBS ONLY SENDS A SINGLE INVOICE FOR THE ENTIRE SEASON OF CLASSES.

PBS CANNOT ACT AS A GO-BETWEEN IN ANY REIMBURSEMENT ISSUES THAT ARISE. IF THE FI DOES NOT SEND PAYMENT WITHIN 30 DAYS FROM THE END OF THE SEMESTER, THE RESPONSIBLE PARTY WILL BE INVOICED BY PBS.

PBS DOES NOT SHARE PER-CLASS ATTENDANCE INFORMATION WITH ANY OUTSIDE PARTIES. EACH STUDENT WILL GET A CERTIFICATE OF COMPLETION AT THE END OF THE SEMESTER.
