PARENTS/GUARDIANS ACKNOWLEDGMENT

Please be advised the West Morris Regional Board of Education will be providing our Parent/Guardians the OPTION of having their son/daughter utilize district transportation to and from School-sponsored events or activities. Therefore, all parents/guardians seeking to take responsibility for their child(s) transportation to and/or from school-sponsored events or activities MUST COMPLETE THIS FORM.

* This form MUST be completed if you're planning to drive or arrange transportation for your child(ren) to or from even (1) away contest.

* This form must be completed at the start of every season in which your student-athlete(s) participate.

* One form must be completed for each child participating.

* An email MUST be sent to the Athletic Director & Head Coach when your son/daughter will not be utilizing district transportation at least 24 hours prior to the scheduled event.

* NO STUDENT IS PERMITTED TO TRANSPORT THEMSELVES OR ANY STUDENT-ATHLETE TO OR FROM AWAY EVENTS/CONTESTS.

I acknowledge and understand that the West Morris Regional High School District will be providing Extra-Curricular Transportation, but prefer to arrange the transportation for my son/daughter as the Parent/Guardian, if needed. *

I acknowledge and understand that as the parent/guardian who is allowing my son/daughter to participate in an Athletic program that I will take full responsibility to ensure my son/daughter has transportation to and from AWAY contests. If I select this option, I will make sure to inform the coach of the games I will be arranging transportation for my son/daughter.

I hereby release, waive, discharge, hold harmless and covenant not to sue the West Morris Regional High School District, its individual members, officers, agents, servants, or employees (hereinafter referred to as the “Board”) from any and all liability, claims, demands, actions, and causes of action whatsoever arising out of or related to any loss, damage, or injury, including death, that may be sustained by my minor child (identified above), myself, any third party and/or any property damage to my child, myself and/or any third party as result of, or in any way arising out of my child traveling to and or from School-sponsored activities in a vehicle not owned or operated by the Board.
