2023-2024 COURSE WAIVER FORM                        DUE BY AUGUST 20, 2023
By completing this form, I (parent/guardian) am requesting my child to be enrolled in a course for which they didn't receive a recommendation for the 2023-2024 school year. I realize that this decision is not the recommendation of the school; however, I, as a parent/guardian, will assume the responsibility for overriding the school’s recommendation.  I am aware that if my child encounters difficulty with this course, a change to a lower-level course may be difficult because of course enrollment limits or scheduling constraints.  I am also aware of the schedule change policies of the school as outlined in the CHS Program of Studies.  Additionally, my child must remain in the course for a minimum of five weeks (scheduling window #3) prior to dropping levels.

A separate form must be completed for each course. 

Students are responsible for checking the CHS website for summer assignments that may be listed as requirements for advanced level courses.

School Counselors will contact the students once schedule changes are complete in Powerschool.

ALL COURSE WAIVER REQUESTS MUST BE SUBMITTED PRIOR TO ANY SCHEDULE CHANGE.
