I, (print name below), acknowledge and understand that any food I receive from Good Shepherd Lutheran Church’s Pet Food Bank program is at my own discretion, that the Pet Food Bank program is supported by private contributions and, thus, Good Shepherd Lutheran Church cannot guarantee the brand, quality, type, freshness, and safety of the food provided to me. I understand that the food may not be the current brand I use to feed my pet, which may upset my pet’s stomach.  I release Good Shepherd, this program (Kiso's Provisions), and its affiliates from all liability that may arise due to the services and products received through Good Shepherd's Pet Food Bank program. I agree not to resell, redistribute, or attempt to return to a store any food or care items received from Good Shepherd’s Pet Food Bank.
