We expect our guardian homes to have the dogs PASS the CGC (canine good citizen) test by 12-16 months of age. Will you be able to do that? *

There may be times that we ask you to  travel with the dog to our home, vet apt, etc. Are you willing to do that? *
Do you understand that if we feel that the dog is not being cared for properly to our expectations, training, grooming, socializing, etc. we can take the dog back into our procession no questions asked.   *

We do ask for updated pictures for our social media would you be able to send updated pictures? *
We like to have open communication with our guardians and we do not want you to feel like you cannot ask us for help, if the pup/dog is struggling with something please come to us first for help. Refrain from going to public form.   *
Is there anything in the home that could harm or alter the health of the dog?  Ex. Alcohol, smoking, drugs. *
Is there any smoking inside home or vehicle where the dog would be? *
Are there any dogs in the home not fixed? *
Do you understand that Blackwater Doodles is responsible for all breedings, breeding related costs, and decisions. *
This dog is only purposed for breeding in the Blackwater Doodles program and will not be bred to any other outside dog without permission from o Blackwater Doodles. Do you understand? *
Any outside breeding that is not a part of Blackwater Doodles contract agreement is voided immediately and dog will be put back in our possession. Do you agree?     *

By putting your name and date on the bottom you are stating that all the information you have provided is correct. During the home check, if we do not feel you are the right fit for our program or information has been falsified, we could terminate this agreement. *
