Patient Verification of Benefits and Responsibility Estimate for Pregnancy and Birth Care
All information entered here stays in a HIPAA protected environment and will only be used to help you understand your benefits and the costs of care.

Many people want to understand the total cost of pregnancy, birth, postpartum, and newborn care.

Verifying benefits is the process of checking to see what your insurance covers.  We can then use your current deductible and policy rules to let you know our best guess as to the cost of your care.  We can also help you  start a payment plan now so that you do not have one lump sum due after care is over.

This process can take up to two business days.

While we do our best to get the most accurate information possible from your insurance company, we cannot be responsible for any alterations in your plan or coverage--please let us know if your insurance changes.

This form can take a moment to complete.  If you have any questions, please call us at 206-712-7352 or email info@MyRVCC.org.  We are happy to assist you.

Next Steps:
1.  Call or lookup in your insurance portal to confirm your insurance is current and active

2.  Do the same for any secondary insurance coverage you may have

2.  Complete the form below

3.  After we have verified your benefits, we will send you a report in your Client Portal or call you if you are not yet an active client.
--This report will include your rate of coverage, deductible information, and whether you are covered for home
and/or birth center births

4.  We do not check your estimates for hospital fees.  Each hospital has different fees and you can call them and ask about their fees and insurance coverage for birthing in their facility.

5.  We do not check your estimates for birth center fees.  If the midwife has confirmed for you that they have privileges at the birth center you want to birth at, you can call there and ask them about their fees and what kind of coverage you might expect from your insurance.

6.  We offer a Financial Assistance Plan (FAP) that you may qualify for. To inquire about our FAP, please wait until we send you back the report and then email Biller@MyRVCC.org.  We do not want financial barriers to limit your options for birth and will do what we can to help you achieve your healthcare goals.
