I understand and agree that in participating in any dance class, workshop, rehearsal, or performance, there is a possibility of physical injury or death. I voluntarily agree, therefore, to assume all risks and responsibility for any such injury or accident, which might occur to me or my child(ren) during any of the WARRENTON BALLET CENTER, LLC AKA WARRENTON BALLET CENTER and STRESS BURNER YOGA classes, rehearsals, performances, or activities. I also exempt, release, and indemnify WARRENTON BALLET CENTER, LLC AKA WARRENTON BALLET CENTER and STRESS BURNER YOGA, its owners, agents, volunteers, assistants, employees, guest artists, faculty members, and/or students from any and all liability claims, demands, or causes of action whatsoever from any damage, loss, injury, or death to me, my child(ren), or property which may arise out of or in connection with participation in any classes or activities conducted by WARRENTON BALLET CENTER, LLC AKA WARRENTON BALLET CENTER and STRESS BURNER YOGA. I further hereby voluntarily agree to waive my rights and that of my heirs and assigns to hold WARRENTON BALLET CENTER, LLC AKA WARRENTON BALLET CENTER and STRESS BURNER YOGA, its owners, agents, volunteers, assistants, employees, guest artists, faculty members, and/or students liable for such damage, loss, injury, or death. I understand that I should be aware of my physical limitations and agree not to exceed them. If I am signing this waiver for my child(ren) under the age of 18, I certify that I am the parent or legal guardian and have the right to waive these rights.

Permission is granted to WARRENTON BALLET CENTER, LLC AKA WARRENTON BALLET CENTER and STRESS BURNER YOGA to use photographs of students/participants for publicity purposes consistent with the mission of WARRENTON BALLET CENTER, LLC AKA WARRENTON BALLET CENTER and STRESS BURNER YOGA. I hereby assign full copyright of photographs and video/audio recordings of me/my child(ren) taken during class administered by WARRENTON BALLET CENTER, LLC AKA WARRENTON BALLET CENTER and STRESS BURNER YOGA, and/or guest artists, or while rehearsing and/or performing on behalf of WARRENTON BALLET CENTER, LLC AKA WARRENTON BALLET CENTER and STRESS BURNER YOGA to WARRENTON BALLET CENTER, LLC AKA WARRENTON BALLET CENTER and STRESS BURNER YOGA with the right of reproduction either wholly or in part. I understand and give my permission to use my/my child(ren)'s name, likeness, image, voice, and/or appearance represented in any illustrations, pictures, photos, video/digital/audio recordings, digital images, taken or made on behalf of WARRENTON BALLET CENTER, LLC AKA WARRENTON BALLET CENTER and STRESS BURNER YOGA. These uses include, but are not limited to, illustrations, bulletins, exhibitions, film, videos, reprints, reproductions, publications, advertisements, and any promotional or educational materialism, any medium now known or later developed, including the use of the publicity materials on the Internet. I understand that I will not receive any compensation for the use of the publicity materials and hereby exempt, release, and indemnify WARRENTON BALLET CENTER, LLC AKA WARRENTON BALLET CENTER and STRESS BURNER YOGA from any and all liability or claims pertaining to use of publicity materials.
