I authorize Big Fish Academy, LLC to electronically debit my bank account according to the terms outlined below. Starting the Monday Oct 5th, 2020 (or the day I submit this form if after that date) which pays for month of October tuition. Your bank account will be debited the below selected amount(s) on the 5th of each Month for the number of Mondays in each month. I understand that December is the only prorated tuition month and it will bill for 2 weeks tuition. I understand that all tuition must be current when the autodraft starts. I acknowledge that electronic debits against my account must comply with United States law.
