The Dissociative Disorders Alliance (DDA) is aspiring to be the UK’s national charity for people with complex dissociative disorders, their allies and practitioners working in the field. We believe you will know best about what you need! So please help us in these early stages of your charity's development by completing this questionnaire. We are looking forward to hearing what you would like to share.

The DDA is very aware of how important safety is for people with lived experience of complex dissociative disorders. Please be reassured that completing & submitting the questionnaire is totally anonymous. This is the case whether you are signed into Google or not - you may see your email address below, this simply means that you are signed into Google, you will see underneath your address it says ‘not shared’. We have ensured that there is no chance of your email being stored.
